Every company that requests an oversize/overweight permit is required to file proof of insurance with Missouri DOT Motor Carrier Services by February 28, 2009. Carriers who do not provide an insurance certificate or Form E will be denied permit privileges until a document of proof is filed. Missouri must also be named as a certificate holder on the insurance certificate. The MO state office address must also appear on the certificate as detailed below. Insurance companies must send a copy of the insurance certificate to MODOT Motor Carrier Services by mail, e-mail, fax, courier, or in person. Form E documents may be submitted electronically.
Insurance must be in force for the duration of each permit period.
